  • Each failure mode recurs across domains because it exploits the same structural gap: override availability.
  • ACP principles do not optimize outcomes; they constrain actions.
  • The goal is not error elimination, but error containment before legitimacy collapse.

Use Guidance

This table should be used:

  • during system design reviews,
  • during governance audits,
  • when evaluating “exception” mechanisms,
  • when assessing whether management authority is substituting for structure.

If a failure mode does not map cleanly, it is likely not yet understood structurally.


Conclusion

ACP does not introduce new managerial wisdom.
It removes the conditions under which managerial wisdom is expected to substitute for governance.
