Purpose
To identify structural warning signs that governance is failing or about to fail—before visible harm occurs.

This diagnostic is qualitative but decisive.


Category I — Override Normalization

🚩 Senior leaders can “temporarily” bypass safeguards
🚩 Emergency exceptions lack clear termination criteria
🚩 Overrides are praised as decisive leadership
🚩 Documentation follows action, not precedes it

Interpretation:
Governance is already conditional.


Category II — Managerial Heroics

🚩 Managers rewarded for “making it work”
🚩 Compliance described as “flexible in practice”
🚩 Institutional pride in handling crises informally
🚩 Success stories depend on bending rules

Interpretation:
The system requires heroism to function.


Category III — Evidence Substitution

🚩 Compliance demonstrated through reports, not artifacts
🚩 Audits rely on explanations
🚩 “Trust us” appears in official responses
🚩 Verification is retrospective

Interpretation:
Legitimacy is narrative, not structural.


Category IV — Scope Ambiguity (AI / Tech Systems)

🚩 Systems reused beyond original authorization
🚩 “Responsible use” language without enforcement
🚩 Scope creep discovered incidentally
🚩 Expansion justified as minor or obvious

Interpretation:
Capability is outpacing governance.


Category V — Crisis Doctrine

🚩 Governance suspended to preserve outcomes
🚩 Crisis success defined as speed, not legitimacy
🚩 No record of refused actions during crises

Interpretation:
Collapse conditions already exist.


Category VI — Silence Around Refusal

🚩 No examples of actions the system refused
🚩 Refusal framed as failure rather than success
🚩 Managers embarrassed by blocked actions

Interpretation:
The system is not allowed to say no.


Diagnostic Outcome

  • 0–2 categories triggered:
    Monitor closely. Structural reform still feasible.
  • 3–4 categories triggered:
    Governance degradation underway. Intervention required.
  • 5–6 categories triggered:
    Legitimacy collapse imminent or already occurring.

Closing Warning

Institutions do not collapse when rules are broken.
They collapse when rules become optional.

Red flags appear long before failure is visible.
Ignoring them is itself a governance decision.
